QUALIFICATIONS FOR CGM

– DIAGNOSIS OF TYPE 1 OR TYPE 2 DIABETES
– THE FREQUENCY OF TESTING MUST BE 4X DAILY AND DOCUMENTED IN MEDICAL RECORD
– FREQUENT DOSING OF INSULIN MUST BE MORE THAN 3X DAILY AND DOCUMENTED
– FREQUENT ADJUSTMENT OF DIABETES REGIMEN

QUALIFICATIONS FOR INSULIN PUMPS

– MUST HAS COMPLETED A COMPREHENSIVE DIABETES EDUCATION PROGRAM
– MUST HAVE DIAGNOSIS OF DIABETES
– MUST BE INJECTING AT LEAST 3X PER DAY AND ADJUSTING INSULIN DOSE
– LAB WORK
– MUST BE TESTING 4X PER DAY (OR ON CGM) AND MEET ONE OF THE FOLLOWING:

  1.   HbA1C greater than 7%
  2.   History of recurring hypoglycemia
  3.   Wide fluctuations in blood glucose before mealtime
  4.   Dawn phenomenon with fasting blood sugars frequently exceeding 200mg/dl
  5.   istory of severe glycemic excursions
